I’m Nick Bischoff, a board-certified psychiatric nurse practitioner. I evaluate you, I prescribe, and I sit beside you as your therapist in every session.

Why I do both

I’ve worked inside the fifteen-minute model, and I’ve seen what it does to patients who get medicated instead of understood, and to clinicians who know better but don’t have the time to practice differently.

My work starts from a different premise: you are not broken. You are healing from what has happened. That means looking honestly at whether a medication is still helping, treating the nervous system rather than just a diagnosis, and making sure you understand every option in front of you before anything changes.

Ketamine-assisted psychotherapy fits that premise. The medicine can quiet the part of you that’s been bracing for years and loosen patterns that insight alone hasn’t moved. In my experience, though, the medicine is the catalyst. What lasts comes from the preparation beforehand and the integration afterward.

The medicine opens the door. The therapy is what walks through it.

What the dual role means for you

Most ketamine programs split the work between a prescriber and a therapist, who may never be in the same room. When one person holds both roles:

  • Your dose is chosen by the person who will be sitting with you, and who can adjust it with your experience in mind.
  • What surfaces in a session is met by someone who knows your full history, including your medications.
  • Medication decisions stay connected to the therapy, including tapering when it makes sense.
  • You tell your story once. No coordinating between offices, no gaps.

How I work

My therapy is trauma-informed and somatic-informed. It goes beyond talk, into where emotion and experience are held in the body. In a medicine session that means tracking what’s happening in you rather than steering it. I’m there to help you stay with what arises and make sense of it, at your pace.

I also specialize in careful medication tapering. Coming off psychiatric medication is an emotional experience as much as a medical one, and for some people ketamine-assisted therapy becomes part of that path. It’s a goal we can plan toward, never a promise, and any taper happens on its own careful timeline.

Nothing gets rushed, and nothing happens without you understanding why.

Training and background

I’m a board-certified Psychiatric–Mental Health Nurse Practitioner (PMHNP‑BC), licensed as an APRN in Kentucky and Colorado, with advanced training in ketamine-assisted psychotherapy. I also teach psychiatric nursing at the university level.
